This is a picture of South African AIDS-activist Zackie Achmat. Achmat is the leader of the Treatment Action Campaign (TAC). Achmat has fought and won several battles to make antiretrovirals more accessible to those who can't afford to pay for them in South Africa. Extensive lobbying by the TAC helped compel large pharmaceutical companies to back down from their legal challenge against South Africa's drug laws, in an attempt to prevent the production and importation of generic Aids drugs.
