The idea is that the word AIDS is an iceberg, and the tip of the letter A is above the water, thus demonstrating that the levels of AIDS in 1987 were just the tip of the iceberg. The text below is a transcript of the voiceover that accompanied the advert.

"There is now a deadly virus that anyone can catch from sex with an infected person. But you can't always tell if someone is infected. The people who have died so far will be the tip of the iceberg. So protect yourself; it's safer if you use a condom. And remember, the more sexual partners, the greater the risk."
